To register simply print off (below) or collect a registration form and new patient questionnaire, fill them in and hand them into the surgery. You will need to provide photographic ID. After you have registered Practitioner Services will automatically recall your medical records from your previous practice and they will be sent to us.
It is our practice policy not to refuse to register anyone on the grounds of race, gender, age, religion, background, disability or medical history. We may, however, refuse to register anyone who is or has been abusive to staff in this or other practices, or whose behaviour is not acceptable or conducive to a bond of mutual trust and respect.
Government guidelines state that all persons over the age of five years must be offered a pre-registration medical. All patients registering with the Practice are asked to see the practice nurse within the first month for a simple health check, including blood pressure and urine testing. You will be asked for details of your past medical history and your current medication will be reviewed, so please remember to bring all current medication along with you, including inhalers and contraceptive pills.

Registration Policy
In accordance with current guidance to prove NHS eligibility all new patients are requested to present two forms of identification. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.
One form of identity must ideally be photographic such as:
- Passport
- Drivers Licence
- Official ID card from Public Services body
- Student matriculation card (current year)
Other documents for prove of residency that are acceptable are:
- Recent utility bill (within last 3 months)
- Council Tax document
- Television Licence
- Payslips (last two months)
- Rent book/agreement (Public Body or Private Landlord)
- Bank Statement (Name and address section only required)
- Solicitors Letter- (Clearly showing name and address)
If unable to provide photographic evidence then you must present one of the above and one of the following:-
- Birth Certificate
- Marriage Certificate
- DivorceAnnulment Papers.
N.B. The above list is not exhaustive and staff are entitled to ask for further proof if required. Please note you will not be registered with the practice if you cannot provide the documents required to prove NHS eligibility.
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Registration With The Practice
Please call in to the Practice and ask for a Registration Form. When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.
Once you’ve been accepted as a patient, your medical records will be transferred to the Practice..
Your medical records may take several weeks to arrive at the Practice.
Children under six years – information regarding childhood immunisation for children under six years should be provided. Please bring along the vaccination card/record if possible.
Patient choice – Patients may choose to see a female doctor or nurse or a male doctor. Please state when arranging your appointment if you have a preference.
Practice Boundary
From Dutch House roundabout it follows the A77 north past Symington and Bogend Toll and then leaves on the B7038 towards Kilmarnock. It immediately then goes left onto the road through Earlston and follows this road until it meets the A759 near Gatehead. It then follows the A759 north through Gatehead and then leaves on the unmarked road from Gatehead to Torntoun joinging the B7081. It includes the whole of the village of Gatehead. It follows the B8081 through Springside going towards Dreghorn, it incorporates the whole of the village of Springside. It follows the B7091 through the middle of Dreghorn to Corsehill roundabout. It also incorporates the whole village of Dreghorn. From Corsehill roundabout it follows Long Drive south through Shewalton and then follows the B7080 until it meets the roundabout on the A78. It follows the A78 back to Dutch House roundabout.
